Endodontic treatment has as its overall goal the long-term retention and restoration of the endodontically treated tooth, including an absence of periapical infection. For this to be achieved, a number of steps and a careful technique are required during instrumentation and irrigation for cleansing and shaping of the canals, and during root canal obturation. Over time, root canal instrumentation options increased to include NiTi reamers and files, Gates Glidden drills, and other designs for manual and rotary use. Instrumentation options and techniques for obturation also increased to include traditional lateral and vertical cold condensation techniques, as well as techniques utilizing carriers of different designs and new materials. Regardless of which combination of instrumentation and techniques is used, successful endodontic therapy relies on a number of sequential steps that must be thoroughly carried out. Endodontics: Part 5 Basic instruments and materials for root canal treatment

In this part the basic endodontic instruments necessary for effective root canal treatment are described. The properties of, and manufacturer's claims for, new instruments and techniques may be compared to these basic principles before they are purchased and introduced to clinical practice. Having the correct instruments for different clinical situations may make treatment both more efficient and more effective. ● A basic pack of all the endodontic instruments required must be available suitably sterilised to ensure rapid efficient treatment. ● Modern radiographic techniques facilitate swift diagnosis and treatment procedures. ● The development of endodontic instruments from reamers to greater taper nickel titanium files is considered. ● The importance of thorough and efficient irrigation with appropriate antiseptic agents is discussed, together with the necessary precautions.

Clinical relevance of standardization of endodontic files dimensions according to the ISO 3630-1 specification

The aim of this study was to examine the current status of standardization of endodontic instruments. Measurements of instrument dimensions were conducted on stainless steel H- and K-files (sizes 08-40), as well as on rotary-driven nickel titanium (NiTi) files (sizes 15-40), to determine the incidence and degree of deviation from ISO 3630-1, 1992 specification. In addition, the dimensions of sets of H- and K-files (sizes 08-25) were measured to identify discrepancies in dimensions among instruments within the same size per manufacturer. The percentage of difference in width were used to determine the graduation from each size to the next. None of the files tested complied with the ISO nominal size. All files were within the ISO tolerance limits. However, under such tolerance limits, there is a high possibility of either sizes overlapping or of great differences between two sequential sizes. Usage parameters of nickel-titanium rotary instruments: a survey of endodontists in the United States

Usage of U.S. endodontists regarding rotary root canal instruments is not well-documented. Therefore, the aim of this study was to retrieve data from a representative sample of endodontists regarding contemporary usage of rotary instruments. A questionnaire addressing (1) nickel-titanium (NiTi) rotary instrument usage and (2) expectations for future instrument design was constructed. An online survey was e-mailed to 954 endodontists in California (n = 358), Iowa (n = 25), Massachusetts (n = 120), Minnesota (n = 49), New York (n = 217), Texas (n = 144), and Wisconsin (n = 41). Data were collected from respondents by using a web-based interface during a 4-month period. Valid responses were received from 359 endodontists, representing a 38% response rate.
