An insightful article from Mckinsey concerning Value Based Care (VBC) and its future. There is an exceptional amount of quality detail in the article, some points that are of particular interest: 1) CMS issued a goal to have all Medicare beneficiaries in an accountable care relationship by 2030. There is a significant opportunity for companies to support providers and help them accept more risk (full and partial capitated). 2) Certain markets will lead the way in proving out VBC models. For example, “based on data from Definitive Healthcare and California Department of Managed Health Care, it is estimated that 90 percent of Southern California’s commercial and Medicare lives are in value-based contracts, as well as nearly 50 percent of its Medicaid lives, making this one of the more mature markets nationally.” California and Florida are two markets that will lead the way with successful providers establishing a strong presence with payers. 3) Value additive advanced analytics will become important in identifying drivers of patient churn and helping providers be proactive in developing mitigating interventions accordingly.Patterson approach with its clients is to assist with understanding the various stages of VBC, solutions required and account development processes to help serve the transition to VBC.Offers programs to work with payers, CMS (Medicare & Medicaid), health systems and providers of care using at risk models and or tenets of VBC.Contact is at dpatterson@pattersongroup.com or call 317-341-3589https://lnkd.in/gdCQNEBN

    Beckers recently interviewed 86 health system executives and asked them about innovations they are launching. While there were many different answers, there were a few that stuck out.·One is the ability for AI to mine data, pick out social determinants of health and then send such detail back to clinical teams at the point of care (Endeavor Health).·Another answer that is important to out of hospital care is the development of virtual connected care teams that use registered nurses and a nursing assistant or LPN to coordinate care delivery outside of the hospital (Trinity Health and Nashville General Hospital).·Using case managers and social workers to progress the patient, manage cost of care and plan safe discharge transitions (Christian Hospital Northeast, St. Louis)These initiatives are aimed at reducing ER visits and hospital readmissions, a major cost and utilization driver. Through initiatives like these the system is evolving to keep individuals out of the hospital and reduce costs.Here are some companies doing important work. A company working in the AI space is Lirio, they provide hyper personalized engagement using AI and behavioral science. A company working in the social determinants of health space is US Hunger, through their social needs survey they are able to collect social determinants of health on populations. Combining AI and social determinants are a great way to engage individuals.     Recently we have posted about SDOH and how they provide insight into everyday circ*mstances and individual health of people. Additive to this we’ve recognized the potential for use of AI that combines Precision Nudging™ with the science of behavior change and artificial intelligence. One company emerging in this field, Lirio with its precision nudging capability allows for individual engagement based on data processed through their AI engine.By having SDOH paired with advanced AI, the AI would be able to learn more thoroughly about each individual and provide the correct engagement for each person in order to close gaps in coverage and care for such needs as increase medication adherence and or patient nutrition.Over the next few weeks we will bring actual uses of AI and confirm and define how patient engagement and care are enhanced delivery improved clinical, operational and total cost of care outcomes.

    In our work over the last several months it's become clear that SDoH must be more a part of successful delivery OOH Care including reduction of ED and hospitalizations. No longer will only one piece of the care solution support improved outcomes. There's some extraordinary work in this space that must be an integral part of providers care plans and further supported by the payer community. Over the next two weeks or so we will feature organizations and providers who are using this service to improve lives of patients & the respective care givers.

    Understanding a persons Social determinants of health can be a difficult process. A new study shows how generative AI can help sift through EHR notes to identify certain SDOH.     Some interesting advances are being made in the medical field with artificial intelligence. Recently, Oracle has integrated its generative AI into Cerner EHR. The generative AI will help clinicians free up time for interacting with patients and help deliver better outcomes. The AI will automate note taking as well as generate context aware next actions such as ordering labs and medication or scheduling follow up appointment.As the healthcare system looks forward hopefully AI is able to help optimize resources and help provide quality care for everyone. Epic is also rolling out generative AI with startup Abridge and Google is also collaborating with Meditech to bring generative AI.     Its important every now and then to bring it back down to earth and ask ourselves, what are the basics of a healthy life style? Everyone has a different starting place on their health journey and everyone’s healthy lifestyle will look different. Some people should simply start walking for 30 minutes a day. Some people should eat more servings of fruits and veggies. Some will do both. One thing is a fact, every attempt towards better wellness habits helps.     Continuing on from our post last week concerning Value Based Care, we wanted to explore what the evolution of the US healthcare system could look like in the next 5-10 years.
